No More Deaths at the hands of the State

The general election is only a week away. As candidates come to knock on your door and seek your vote, there is no better opportunity to ask them what they will do to address the growing number of preventable deaths at the hands of the state.
If recommendations were made following the death of your loved one, aimed at protecting others, would you expect to see them enacted?
Public and private bodies have a duty to keep us safe from harm. Yet the UK fails to monitor what happens to life-saving recommendations made following deaths in state detention as well as other areas of public importance such as fire safety and NHS failures.

There is no central oversight of recommendations and poor transparency on what, if any, changes have been made.
Important recommendations are left to gather dust and preventable deaths continue.
INQUEST is calling for a National Oversight Mechanism: a new independent public body responsible for monitoring recommendations arising from inquests, reviews, inquiries and investigations following state-related deaths.

• Collate recommendations and prganisations’ responses in a new national database
• Analyse responses from organisations and issue reports
• Follow up on progress, escalate concerns and share thematic findings
This election is an opportunity to drive change and save lives.
